  1. I managed "fix" this by modify rsreportserver.config file, not an official fix, but at least it works for me. I wrote a blog post about it. Hope it helps you too. http://www.thesccm.com/sccm-1606-sql-2016-report-subscription-failed/

What step do you have after Upgrade or Install Operating System? base on the test2_smsts.log: Successfully completed the action (Upgrade Operating System) with the exit win32 code 0, "Expand a string: smsboot.exe /target:WinPE., "A valid boot image ID must be present in order to boot into WinPE" Seems it try to boot to winpe after install/upgrade operating system. So can you double check you "Restart Computer" step? It should be set "restart to operating system", not "restart to winpe"

Have you check out this https://www.windows-noob.com/forums/topic/13917-how-can-i-use-the-upgrade-task-sequence-in-system-center-configuration-manager-current-branch/?p=53402, you can use this to make a inPlace upgrade your existing workstations to 1511, and you need another TS to deploy OS to new workstations.
  4. Do you have internet Access in your server? It needs connect to Microsoft to download in console updates, and make sure you have "Servicing Connection Point" role installed, and setting is "Online".
  5. Failed to get information from MP, can you check if the MP list can be opened? http://your_sccm_server/sms_mp/.sms_aut?mplist
  6. Feature Update to Windows 10 Enterprise, version 1607 show up in my SCCM, but my test machine which has install Windows 10 Enterprise version 1511 did want to get this version 1607, SCCM report said this update is not required to my test machine.
  7. Hotfix is out. https://support.microsoft.com/en-us/kb/3127032 Now I can download those upgrades, but they doesn't show up in Software Center, other updates are ok.
